The below benefits are optional and can be added onto the core plans of Extended Health & Dental plan.
Allows employees to insure their spouse or children under their plan. Spouse can be insured for $10,000 and each child up for $5,000.
This coverage pays out a lump sum benefit on the diagnosis of one of the covered illness. Once a plan member has been diagnosed, survives the waiting period they will receive a lump sum benefit. Below you will see a partial list of the covered conditions include:
Critical Illness is priced individually.
Long Term Disability (LTD) pays a benefit amount to replace a portion of a plan member’s income while faced with long term absence from work due to either illness or injury. Percentage of monthly income it can be replaced up to is 66.67% to a max of $8,000 / month. Coverage lasts to age 65. Every $100 of LTD added to the plan is rated at a fixed cost.
Vision Care Coverage
Vision plan includes glasses, contact lenses, laser eye surgery. Adults 18 + years old are eligible for $250 every 24 months. Children under the age of 18 are eligible for $250 every 12 months.
Includes major restorative services such as Crowns, Surgical Excision, Transplantation, Re-positioning. T here is a 50% coinsurance with maximum benefit per person per year of $1,500. Major Dental is only an optional benefit to be added to Gold and Silver level plans.
Employee Assistance Program
Employee Assistant Program (EAP) benefit provides plan members with access to confidential and voluntary support services that can help address variety of problems in their lives. EAP provides a range of support services such as legal, financial, family and workplace. Examples are stress, anxiety, mental health issues such as depression, addictions, family problems and nutrition challenges. This is a great way to let employees know you have equipped them with the resources to help deal with serious issues. There is a fixed rate per person added to the plan.
